Background

Population Explorer is a powerful population health tool within the HIE Portal that brings data together from across the HIE network to help organizations identify opportunities to improve patient care and strengthen outreach efforts. 

The tool enables users to create targeted searches, apply filters, track outreach activities, and receive near real-time notifications when patients experience healthcare events. These capabilities can help organizations identify patients who may benefit from additional support, monitor the effectiveness of outreach efforts, and take timely action when care opportunities arise.
