Clinical Information

Our HIE Portal provides a comprehensive clinical data application with real-time access to key medical information. Preloaded patient data, including overdose alerts, encounters, labs, radiology reports, care summaries, immunizations, and problem lists, is available directly within the provider’s workflow—no extra login or navigation required. 

This section includes the patient’s demographic profile and lists next of kin. It also provides alternate contact information, if available.

Clinical Data includes overdose alerts, inpatient and outpatient encounters, labs and radiology reports, summaries of care, and separate consolidated lists of the patient’s immunizations and problems.

Medication Management consolidates medication information from a patient’s continuity of care documents (CCDs) into a single, centralized location, streamlining access and making it easier to compare and review medications quickly.

This section includes the patient’s Care Team, along with details about the patient’s care program and manager. Care Alerts provide high-priority information for complex patients. Referral History displays referrals made to organizations addressing the patient’s social needs, including outcomes, sender, and recipient information when available. Advance Directives can be located on MyDirectives.com and the WV End of Life (EOL) Registry.

This section includes highlights when a patient has reported to a provider that they experience problems related to social issues known to impact health using z-codes.

PDMP

The Prescription Drug Monitoring Program (PDMP) tracks the dispensing of drugs containing controlled dangerous substances (CDS). Its primary goal is to help healthcare providers, pharmacists, and public health professionals identify and prevent prescription drug abuse. At the same time, the PDMP ensures that prescription data is used responsibly, supporting both the integrity of medical practice and patient access to necessary medications.

Population Explorer

Population Explorer is a user-friendly application within the HIE Portal that allows clinicians and care teams to monitor patient encounters, providing real-time notifications for hospital visits and a six-month encounter history. Powered by the CRISP Event Notification Delivery (CEND) technology, Population Explorer ensures seamless access to up-to-date patient data directly from the HIE Portal. 

Key features include:

  • Access & Review: View your patient panel and export recent encounters. 
  • Filtering: Filter encounter data based on your specific needs, including Inpatient, Emergency, Z-code, Diagnosis code, Facility type, and Date range. 
  • Intra-Portal Navigation: Seamlessly access clinical data, and prescription drug monitoring (where available) directly from the application widget in your HIE Portal. 
  • Customizable Alerts: Tailor notifications to meet specific clinical needs, helping to manage patient populations more efficiently and improve patient care coordination. 
  • Enhanced Care Coordination: Use CEND to facilitate care transitions, meet quality standards, and capture revenue opportunities.
